ProHealth Physicians has transitioned to a new Electronic Health Record (EHR) system, Epic. This includes a new patient portal called MyChart. To view your ProHealth medical history, you'll need to register for an account.
Signing up is simple
There are many ways to sign up for MyChart. If you use FollowMyHealth, you will have to sign up for MyChart to stay connected with us.
Once you are registered with a ProHealth account, you can sign in and view your health information at any time on your computer or smartphone. For easy access, download the MyChart app in the app store for your iPhone or on Google Play for your android.
You may get an enrollment email with a sign-up link. This link is your access to register for a MyChart account. You may also receive an activation code via email.
In person
If you don't have an email address and wish to register for a MyChart account, you can sign up with our reception staff in the office. No appointment needed.
Online
Didn't get an email? Complete the online form and submit it. Due to the high volume of requests, it may take several days to verify your account.
You might be familiar with MyChart, but if you're not, here are a few key features. You can:
- Review your complete health history. You will have access to all your medical records, including information on:
- Diagnoses
- Medicines
- Allergies
- Immunizations and more
- Schedule appointments. Manage appointments with just a few clicks, saving you time and hassle.
- Request prescription refills. We'll have them sent directly to your preferred pharmacy.
- Check test results. View your lab and test results as soon as they're ready. This empowers you to stay informed about your health.*
- Connect with your health care team through secure messaging. Ask questions, seek advice, and get timely responses.**
