Closing: Ear, Nose & Throat (ENT) and Hearing & Balance 

Changes to our specialty services

As of Nov. 2, 2023, ProHealth Physicians Ear, Nose & Throat (ENT) and Hearing & Balance locations will permanently close. We will not offer any ENT or audiology services after this date. You will be contacted if this change affects an upcoming visit or procedure.

If you have a ProHealth Physicians primary care provider, they are still available to care for you.

We encourage you to keep seeing a specialist for your care

You may be able to stay with your current ENT specialist or audiologist.

They’re in the process of setting up a new medical group that will open at a future date in similar locations. Their contact information is listed below.

You can choose the specialist you want to see. Check with your health plan to find audiology care providers in your network. We've also included contact information for other ENT groups below.

Give us permission to release your records

Please fill out and return this HIPAA-compliant medical record authorization form to us as soon as possible. This allows us to send a copy of your medical records to you or the specialist of your choice. Our address is listed on the back of the form.
